Abstract

Objectives: To determine the prevalence of FLT3-ITD and FLT3-D835Y using multiplex PCR combined with fragment analysis and compare detection with next-generation sequencing (NGS).

Results: The study included 154 newly diagnosed AML patients at the Blood Transfusion and Hematology Hospital from January to June 2026. Multiplex PCR combined with fragment analysis detected FLT3-ITD in 21 patients (13.64%) and FLT3-D835Y in five (3.25%); NGS detected these mutations in 12 (7.79%) and five (3.25%), respectively. Nine FLT3-ITD-positive cases were detected only by multiplex PCR combined with fragment analysis, representing 42.9% of cases positive by this method (p < 0.05). NGS additionally identified I867T, D835H and p.Ile836del in three other patients.

Conclusions: In this study, multiplex PCR combined with fragment analysis detected more FLT3-ITD-positive cases than the NGS workflow used, while NGS identified additional variants beyond D835Y. Combining both methods helps avoid missing mutations relevant to prognosis and treatment.
