Abstract

Objective: To evaluate factor VIII activity in fresh frozen plasma (FFP) after preparation and after one year of storage at Da Nang Hospital, and to investigate selected associated factors.

Subjects and methods: This descriptive longitudinal study included 240 FFP units prepared at the Department of Hematology and Transfusion Medicine, Da Nang Hospital. Factor VIII activity, volume, and visual appearance were assessed after preparation and after one year of storage at -18°C to -25°C. Associations with preparation start time and ABO blood group were analyzed, and the reduction in factor VIII activity was compared between two periods of cold storage management.

Results: The mean factor VIII activity was 0.87 ± 0.42 IU/mL, and 78.3% of units met the quality criterion. After preparation, the mean factor VIII activity was 0.94 ± 0.47 IU/mL, with 80.8% of units meeting the criterion; after one year of storage, it decreased to 0.81 ± 0.35 IU/mL, with 75.8% meeting the criterion. The reduction in factor VIII activity after one year of storage was 24.73% in the pre-improvement period (September 2022–September 2023), compared with 4.21% in the post-improvement period (October 2023–September 2024). The proportion of units meeting the factor VIII criterion decreased with increasing preparation time, from 94.9% in the <6-hour group to 53.8% in the 12–18-hour group. Group O FFP had the lowest factor VIII activity and the highest non-compliance rate. Longer preparation times and blood group O were associated with a higher risk of failing to meet the factor VIII quality criterion.
