CHARACTERISTICS OF BASIC HEMOSTASIS AND COAGULATION TESTS IN PATIENTS BEFORE HEPATOBILIARY SURGERY AT 108 MILITARY CENTRAL HOSPITAL
Main Article Content
Abstract
Objective: To describe basic hemostasis and coagulation test characteristics and associated factors in patients before hepatobiliary surgery.
Subjects and methods: A retrospective, analytical cross-sectional study of 177 patients scheduled for hepatobiliary surgery at 108 Military Central Hospital from March to May 2026. Abnormality rates of PT%, rAPTT, fibrinogen and platelet count were compared by age, sex, disease group and clinical bleeding using Fisher’s exact test.
Results: Males accounted for 62.7%, patients aged ≥ 60 for 49.2%, and biliary tract disease for 72.9%. Mean PT%, rAPTT, fibrinogen and platelet count were 105 ± 20%, 1.15 ± 0.36, 3.83 ± 1.18 g/L and 255 ± 94 G/L, respectively. Fibrinogen had the highest abnormality rate (41.8%, predominantly elevated – 36.2%); prolonged rAPTT 16.4%; thrombocytopenia 13.5%; reduced PT% 6.8%. At least one abnormal test was found in 56.5% of patients. Prolonged rAPTT, low fibrinogen and thrombocytopenia differed across disease groups (p < 0.001), being highest in the cirrhosis/liver failure group. Only 4 patients had clinical bleeding; thrombocytopenia showed a trend towards association with gastrointestinal bleeding (OR 13.8; 95% CI 1.1 – 174; p = 0.052), whereas reduced PT%, prolonged rAPTT and low fibrinogen were not associated with bleeding (p > 0.05). Age and sex were not associated with abnormal coagulation tests.
Conclusion: Most patients before hepatobiliary surgery retained preserved basic hemostasis; abnormalities concentrated in the cirrhosis/liver failure group. The trend linking thrombocytopenia with bleeding requires confirmation in larger studies.
