Abstract

Blood transfusion practice is shifting from an approach based primarily on laboratory thresholds and blood component provision toward therapy tailored to each patient’s functional needs, risks, and clinical context. This review discusses the principles and clinical practice of personalized transfusion, including patient blood management, function-based transfusion indications, immunologic risk stratification, context-specific component selection, individualized transfusion dose and rate, response monitoring, and haemovigilance. It also proposes strengthening patient-centered transfusion practice, closer collaboration between hospitals and blood suppliers, data integration, and the use of digital technologies and decision-support tools to improve the effectiveness and safety of transfusion therapy.
