INITIAL RESULTS OF ENDOMYOCARDIAL BIOPSY FOR DIAGNOSIS OF REJECTION AFTER HEART TRANSPLANT AT VIET DUC UNIVERSITY HOSPITAL

Abstract
Purpose: Report and evaluate the initial results of endomyocardial biopsy - EMB for diagnosis of heart transplant rejection at Viet Duc university hospital. Patients and method: This is retrospective, descriptive study of patients, who underwent endomyocardial biopsy in total of 39 heart transplant cases at Cardiovascular and Thoracic center, Viet Duc university hospital. Results: There were three patients, two of them were male. Average age was 43 years. Endomyocardial biopsy was indicated for patients, who had signs and symptoms of cardiac rejection. Pathological results of biopsy showed manifestations of rejection at different levels in all cases. The patients were responded to high-dose Steroid therapy according to the rejection regimen based on the rejection classification of the International Society for Heart and Lung Transplantation - ISHLT 2004: clinical symptom improvement, hemodynamic improvement, discontinuation inotropic medications, intravenous diuretics and discharge after 4 weeks of treatment. There were no serious complications after the biopsy, 1 patient was discovered a coronary fistula 6 months after the biopsy during routine check-up coronary angiography but no obvious clinical manifestations and intervention. Conclusion: The usage of endomyocardial biopsy in the diagnosis of cardiac rejection at Viet Duc University Hospital was helped to confirm the diagnosis and contributing to the effective treatment of post-transplant rejection.
