THE CHARACTERISTICS OF VENTRICULAR ARRYTHMIAS IN PATIENTS WITH HEART FAILURE REDUCED EJECTION FRACTION AT LE VAN THINH HOSPITAL

Abstract
Background: Ventricular arrhythmias and heart failure with reduced left ventricular ejection fraction were strongly correlated. In particular, ventricular arrhythmias increased mortality in heart failure patients with reduced left ventricular ejection fraction. Objective: To determine the characteristics of arrythmias in heart failure patients with reduced ejection fraction. Method: Cross-sectional study. Results: During the study period from Mars to September, 2020, there were 40 cases of heart failure with reduced left ventricular ejection fraction, treated at the clinic of Cardiology and Geriatric Department - Le Van Thinh Hospital. The mean age in the population was 66.70 years. The ratio of male:female was 1.22/1. The rate of ventricular arrhythmias in heart failure patients with reduced left ventricular ejection fraction as measured by 24-hour holter ECG was 82.50%, including monomorphic premature ventricular complex, polymorphic premature ventricular complex, and multifocal premature ventricular complex and nonsustain ventricular tachycardia. The rate of premature ventricular complex was 82.50%. The rate of Lown grade I premature ventricular complex was 37.50%. The rate of dangerous ventricular arrhythmias (Lown III-V grade) was 40%.The rate of ventricular tachycardia is 15%. In the population with premature ventricular complex, the proportion of patients with heart failure NYHA III was 72.50%. In the population with dangerous ventricular arrhythmias, the proportion of patients with NYHA II heart failure was 75%. The mean LVEF in patients with ventricular arrhythmias was lower than mean LVEF of patients without ventricular arrhythmias (28.15±7.89% and 34.57±4.24%, respectively, with p = 0.04). We also compared the mean LVEF in group of patients with dangerous ventricular arrhythmias and group of patients without dangerous ventricular, which were 27.34 ±6% and 29.7±9%, respectively, with p = 0.01. Conclusions: Ventricular arrhythmias were common complication in heart failure patients with reduced left ventricular ejection fraction. The rate of ventricular arrhythmias in the population was 82.50%. The rate of dangerous ventricular arrhythmia was 40%, in which the rate of ventricular tachycardia was 15%. Ventricular arrhythmias and left ventricular ejection fraction were closely related. Decreased left ventricular ejection fraction tended to have a high incidence of ventricular arrhythmias.
