RESULT OF DEBRIDEMENT BY ARTHROSCOPY TO TREAMENT KNEE SEPTIC ARTHRITIS AT THE DEPARTMENT OF ORTHOPAEDIC AND SPINE SURGERY - BẠCH MAI HOSPITAL

Abstract

Knee septic arthritis is a condition in which bacteria penetrate inside and damage the synovial-cartilaginous system, which can lead to irreversible damage, loss funtion of the knee. Treatment of pyogenic arthritis depends on the clinical stage and stage of the disease, which can apply conservative treatment (aspiration and antibiotic therapy) or surgical treatment (irrigation and debridement the synovitis). In order to evaluate the results of arthroscopic surgery for the treatment of knee septic arthritis, we conducted a prospective and retrospective cross-sectional study of 38 patients diagnosed and treated for knee osteoarthritis during the period from May 2019 to April 2021 at the Department of Orthopedic and Spine Trauma - Bach Mai Hospital, the shortest follow-up time was 6 months and the longest was 23 months. Research results: After 6 months of surgery, the VAS score at the knee joint decreased from 8.3 ± 0.78 before surgery to 0.47 ± 0.21 (p<0.001), there were no patients with swelling, redness, warth symptoms in the joints, 4 patients (10.53%) with joint effusion at median level. The mean score of Lysholm scale before surgery 38.89 ± 4.27 to 91.45 ± 3.8 after 6 months (p<0.001). Subclinical: Staphylococcus aureus is the most common  (MRSA 55.26%). The WBC, CRP and ESR returned to normal after treatment. Complications: hemorrhagic knee joint in 2 patients (5.26%), recurrent purulent knee arthritis 1 patient (2.63%), 1 patient (2.63%) had in shoulder septic arthritis. Conclusion: Debridement and Irrigarion by arthroscopic surgery for knee septic arthritis have good results.
