EFFICIENCY OF CHRONIC DISEASE MANAGEMENT AND TREATMENT ADHERENCE OF THE ELDERLY IN HOME HEALTH CARE SERVICES

Abstract
Objective: Evaluation of the effectiveness of Home Health care service of Le Van Thinh Hospital in chronic diseases management and treatment adherence in elderly in 2021-2022. Methods: Prospective descriptive study. Results: The study was conducted on 101 elderly with chronic diseases, 71% were women and 65% were aged 70 years or older. 90% of the elderly have high blood pressure, 40% have diabetes and 47% have varicose veins. Research shows that clinical status improvement, chronic disease management rate increases gradually after 3 times of home health care visit: 64(63%), 77(76%), 83(82%) with p=0.032. In addition, improvement in medication adherence by using home health care service has statistically significant. Conclusion: Home health care service is effective in managing chronic disease and treatment adherence among elderly. However, it has not changed the behaviors such as smoking, drinking alcohol, eating habits.
