KẾT QUẢ SỚM PHẪU THUẬT SỬA TOÀN BỘ ĐIỀU TRỊ THÔNG SÀN NHĨ THẤT BÁN PHẦN Ở NGƯỜI LỚN TẠI BỆNH VIỆN BẠCH MAI GIAI ĐOẠN 2018 – 2021

Abstract

There were 29 patients, including 12 males and 17 females. The average age is 39 ± 13 years old (from 18 to 61 years old). Heart failure symptoms: NYHA II (82.8%), NYHA III (6.9%), NYHA IV (3.4%). 1 patient had NYHA IV dyspnea (3.4%). Echocardiographic results: Moderate and severe mitral valve regurgitation had 21 patients (72.4%), moderate and severe tricuspid valve regurgitation 26 patients (89.7%). Pulmonary hypertension: Mild (41.4%); Moderate (48.3%); Severe (10.3%). The average size of the atrial septal defect is 27 ± 11 mm (from 8 to 51 mm). Cardiopulmonary bypass time was  85 ± 22 minutes (45 to 130 minutes). Crossclamp time was 62 ± 15 minutes (from 34 to 89 minutes). After surgery: NYHA II (10.3%), no patients with NYHA III or IV. Echocardiographic results: 34.5% patients with moderate and severe mitral valve regurgitation. 3.4% had moderate tricuspid regurgitation, have no patients severe tricuspid regurgitation. 1 case with severe mitral valve regurgitation  (3.4%) and needed to be repair surgery. 2 case had to implant a permanent pacemaker (6.9%). There was no hospital mortality and no other serious complications. Conclusion: Total partial repair of the atrioventricular septal defect was safe and effective with low complications. The early postoperative results is good.
