OUTCOMES OF LAPAROSCOPIC SURGERY AND ROLE OF ABDOMINAL DRAINAGE IN TREATMENT OF CHOLEDOCHAL CYST IN CHILREN AT VIETNAM NATIONAL'S CHILDREN HOSPITAL

Abstract

Aim: to evaluate of outcomes of laparoscopic surgery (LS) and role of abdominal drainage in treatment of choledochal cyst (CC) in children at Vietnam National' s Children Hospital. Methods: retrospective study with all the patients were diagnosed of CC and managed by LS at General Surgical Department from 1/2018 to 12/2020. Patients who suffered from any abdominal operation were excluded from this study. The pre- and post-operative data were collected. The data were collected and analyzed by Excel and SPSS 20.0. Results: There were 221 patients in this study: 54male and 167 female patients. Todani's classification reveals: 114 patients in type I and 107 patients in type IVA. The diameter of cyst ranged 10 to 105mm.The patients's age ranged 1 month to 14 year olds. The operative time's mean was 161,8 minutes. Intra-operative complicatios remained one case and one case of conversion. The general complications is 8,1 %. The patients were put drainage in 81 cases. The mean of operative time and hospital stay were 161,8 minutes and 7,55 days, restropectively. The comprasion between 2 group: with (Group A) or without abdominal drainage (Group B) show results: the time of operative time and hospital stay shorter in  group B.  One case of complication was from group A. The post-operative complications rate in group A was higher than group B (persistent abdominal abscess, bilious leakage). The patients in group B show better results than group A. Conclusion:Abdominal drainage of LS for CC does not decrease the risk of post-operative complications but can increase the hospital stay. There is safe in cases of not puting abdominal drainage.
