Abstract

Background: Semi-rigid thoracoscopy is a method that helps to directly observe the surface of parietal and visceral pleura, can easily adjust the tip of the tube, from which it is possible to biopsy the lesions. suspected lesions to obtain the most accurate histopathological diagnosis to help diagnose the cause of exudative pleural effusion. Objectives: To evaluate the results of semi-rigid thoracoscopy for exudative pleural effusion. Subjects and methods: A study of 69 patients with exudative pleural effusion use semi-rigid thoracoscopy at the Respiratory Center of Bach Mai Hospital since November 2021 to August 2022. Conduct analysis of pleural lesions and diagnostic value of semi-rigid thoracoscopy. Results: The mean age of patients was 47.14 years. Tumor lesions and pleural thickening are mainly seen in patients with pleural effusion due to cancer, small nodular lesions and pleural septum are common in patients due to tuberculosis, the difference is statistically significant with p < 0, 05. The general diagnostic value of semi-rigid thoracoscopy is 92.8%. The sensitivity, specificity, positive predictive value, and negative predictive value of semi-rigid thoracoscopy in pleural tuberculosis were 98.1%, 100%, 100% and 92.3%, respectively. The sensitivity, specificity, positive predictive value, and negative predictive value of thoracoscopy in malignancy were 90.9%, 100%, 100% and 98.2%, respectively. Complications was chest pain (62.3%), and 1 patient (1.5%) requiring endotracheal intubation and mechanical ventilation after thoracoscopy. Conclusion: Semirigid thoracoscopy is simple, safe and effective procedure in diagnosing exudative pleural effusion of unknown etiology with high diagnostic accuracy and minor procedure related complications.
