THE RESULTS OF DEFINITIVE TREATMENT FOR HEAD AND NECK CANCER WITH RADIOTHERAPY IN ELDERLY PATIENT AT DA NANG ONCOLOGY HOSPITAL

Abstract

Objectives: To evaluate the response, toxicity and complication of definitive treatment regimens combine with radiotherapy in the elderly patients with head and neck squamous cell carcinoma (HNSCC). Methods: 37 patients with head and neck cancer ≥ 60 years old who received definitive treatment with radiotherapy, including: postoperative adjuvant radiotherapy, definitive concurrent chemoradiotherapy or definitive radiotherapy alone at Da Nang Oncology Hospital from 1 June 2019 to 31 December 2020 with case series study. Results: The majority of age group belong to 60-70 (70%). Male/ female ratio is 3:1. All patients had good performance status index. The histopathology of the patients was squamous cell carcinoma grade 1 (46%) and squamous cell carcinoma grade 2 (43.2%). Mostly patients cancer stage is stage III and IV (65,1%). Dermatitis, mucositis accounted for 91.9%, pain, dysphagia over 90%, hematologic toxicity 8.1-43.2%, no grade 3/4 toxicities. The complete response rate at 3 months is 89%. The rate of recurrence and metastasis at 6 months was 10.8% (4 cases). Complications of leukopenia, dysphagia, pain reduce the rate of treatment response. Conclusions: Elderly patients with head and neck cancer had good performance status, tolerating effective treatment with appropriate regimens with high response rates and side effects within acceptable limits.
