Abstract

Objectives: To evaluate the results of applying nerve integrity mornitor (“NIM”) in transoral endoscopic thyroidectomy vestibular approach (“TOETVA”). Methods: A prospective study on 30 patients with thyroid tumor undergoing laparoscopic surgery using NIM at Hanoi Medical University Hospital from September 2021 to September 2022. Results: The average age was 39.67±7.9, in which female patients accounted for 93.3%. Results of ultrasound presented that the average tumor size was 12.3±10 mm. Postoperative pathology showed differentiated thyroid cancer in 23 cases (76.7%), benign tumor in 7 cases (23.3%) and 8 cases with central cervical lymph node (34.8%). The average surgical time was 100,5±17.3 minutes, of which the mean blood loss interval was 9.0±2.4 ml. A single case was identified with intraoperative recurrent laryngeal nerve injury due to heat generated during the severing of the berry ligament and inferior thyroid artery. During the postoperative follow-up, the patient had hoarseness and recovered after 3 months. The average hospital stay after surgery was 3.63±1.3 days. Conclusion: The application of NIM in TOETVA is proved to be feasible and helps the surgeon determine to determine during surgery the status of the recurrent laryngeal nerve, the mechanism and time of injury, thereby predicting the function of the nerve after surgery.
