THE DISEASES STRUCTURE AT AN BIEN DISTRICT HOSPITAL UNDER THE IMPACT OF THE COVID-19 PANDEMIC IN 2018, 2019 AND 2020

Abstract

The re-prospective study used database from the Health Insurance report (accounting for more than 95% of all hospital visits) in 3 years: 2018, 2019 and 2020 (225,336 outpatient visits/treatment and 127,545 inpatients). With the objectives: (1) Describe the disease structure over 3 years in the whole hospital. (2) Analysis of changes in disease structure in inpatient and outpatient areas over the years in the context of the impending COVID-19 epidemic. Apply descriptive and analytical statistics. The results show that: The overall disease structure of the hospital is quite stable for 3 years. The three chapters with the highest percentage of diseases are Chapter IV - diseases of the endocrine, nutritional and metabolic systems (10-12.9%) Chapter X - respiratory diseases (22.3 to 27.4%) and Chapter XIII - Musculoskeletal system diseases account for the highest rate. Changes in disease structure in both inpatient and outpatient areas over the years in the context of the impending COVID-19 epidemic: In terms of the number of patients, in 2020 is lower than in previous years, the clinic/hospital area has increased sharply, while the inpatient area has decreased deeply, especially for diseases in the Top10. The three diseases that decreased most inpatient treatment were: gastritis (K29) decreased more than 10 times, from 6269 cases to 607 cases, asthma (J45) and atopic dermatitis (L20) also decreased up to half of cases. Conclusions: The fear of the COVID-19 epidemic has affected the flow and structure of patients to the hospital in general and strutures of both inpatients and out-patients.
