OUTCOME OF TOTAL LARYNGECTOMY FOR THE TREATMENT OF LARYNGEAL CANCER IN UNIVERSITY MEDICAL CENTER HCMC FROM 2018 TO 2021

Abstract

Background: Late-stage laryngeal cancer is usually treated with total laryngectomy and postoperative radiation therapy. This surgery is a major surgery, common in elderly laryngeal cancer patients, many comorbidities, and easy complications during and after surgery. Objectives: The aims of this study were to assess surgical results of patients with laryngeal cancer after total laryngectomy and some factors related to recurrence and death of patients. Method: Retrospective study on 64 laryngeal cancer patients undergoing total laryngectomy at University Medical center HCMC from January 2018 to December 2021. Then, monitor the patient's recurrence and survival, perform analysis to evaluate the influence of several factors on the recurrence rate and mortality rate. Result: The majority of laryngeal cancer patients were male (96.9%) whose age ranged from 30 to 89 years old with a mean age of 64.4 ± 10.5 years. The average number of days in hospital after surgery was 12.9 ± 3.8 days (8-28 days). In the postoperative period, complications including surgical site fluid collection and pharyngeal fistula all were 9.4%, wound infection was 7.8%, pneumonia and chylous fistula all were 4.7%, bleeding was 3.1%, tracheostomy stenosis was 3.1%, subcutaneous pneumothorax was 1.6%. All patients were treated and discharged in 100% stable condition. During the shortest follow-up period of 6 months and the longest period of 4.5 years, 9 deaths (14.1%) were recorded, 6 recurrences (9.4%). The mean disease-free and overall survival were 45.3 ± 1.7 months and 42.5 ± 2.2 months, respectively. The disease-free and overall survival rates after 3 years were 88.1% and 82.2%. The overall survival time in patients with lymph node metastasis was shorter than in patients without metastasis (p < 0.001). Conclusion: The results of total laryngectomy combined with chemotherapy and radiation have positive results and are quite effective for patients with laryngeal cancer when conservative surgery isn’t possible.
