Abstract

Background and Objectives: Diabetes mellitus (DM) is a chronic, non-communicable disease, with an increasing number of people infected, leading to a burden of treatment costs. Kien An Hospital in Hai Phong City is a Grade I provincial hospital. Annually, the hospital examines and treats from 100,000 to 110,000 patients, of which thousands have type 2 diabetes. Analysis of treatment cost structure to describe the cost structure (direct medical costs, direct non-medical costs and indirect costs) is necessary for the development of a solution to effectively manage the funding for treatment from the state budget and out-of-pocket expenses of people living in Kien An district and surrounding areas. Study population and methods: Treatment costs for patients with type 2 diabetes who have health insurance are managed on an outpatient basis at the Clinic of Medical Examination Department, Kien An Hospital, Hai Phong City from January 1, 2021 to December 31, 2021. This is a retrospective cross-sectional descriptive study based on outpatient medical records in 2021 and face-to-face interviews with 110 patients. Data processing and analysis using Excel software. Results: Direct medical costs accounted for 95.48% of total costs. In the structure of total direct medical costs for the treatment of outpatients with type 2 diabetes, the cost of self-purchase of drugs and functional foods accounted for the highest proportion (62.23%), the health insurance paid for a highest proportion of 34.49%. The cost of drugs to treat type 2 diabetes accounted for the highest proportion (48.56%) of the total cost of drugs covered by health insurance. Travel costs represent 78.07% of direct non-medical costs. The expenses due to the absence from work of the person accompanying the patient represented 67.43% of the total indirect costs. Conclusion: The average outpatient treatment cost per visit of type 2 diabetes patients treated on an outpatient basis at Kien An Hospital in 2021 is VND 2,429,305 (direct medical costs represent 95.48%; direct non-medical costs represent 3.08%; indirect costs represent 1.44% of the cost structure). Health insurance covers 34.49% of direct medical costs.
