Abstract

Peripartum cardiomyopathy (PPCM) is a rare form of pregnancy-associated myocardial disease characterized by left ventricular systolic dysfunction. Peripartum cardiomyopathy (PPCM) is one of the leading causes of maternal death in European countries. In December, 2021, we recorded a case of PPCM after cesarean section for a 30-year-old woman, IVF, 38 weeks, gene hypercoagulability. The pregnant had no history of medical disease, undergone laparoscopic surgery to diagnose infertility, and there was nothing special about anesthesia. Preoperative examination at 34 weeks showed no signs of pathology. We performed spinal anesthesia for cesarean section, during and after surgery the patient had hypotension that was treated by vasopressors  but blood pressure was still difficult to control. Ultrasound detected acute heart failure, severe preeclampsia. Preeclampsia and PPCM, especially in the last month of pregnancy, are rare, but can be life-threatening, so it should be detected and treated promptly. Here, we report a serious case of PPCM that was successfully managed by a multidisciplinary team led by anesthesiologist.
