EFFECTIVENESS OF THE PROMOTING PERITONEAL DIALYSIS PROGRAM AT THONG NHAT HOSPITAL

Abstract
Objectives: To evaluate the effectiveness of the “Peritoneal dialysis (PD) promoting program” after 2 years of launching. Primary outcomes included the percentage of newly diagnosed ESRD patients who chose PD therapy. Methods: A retrospective cross-sectional study was conducted on 295 newly diagnosed ESRD patients at the Department of Nephrology and Dialysis, Thong Nhat Hospital between January 2020 and June 2022. In this promoting program, we offered a comprehensive consultation and care plan, including: (1) Medical consultations to enhance patients’ understanding of the three modalities of renal replacement therapy (hemodialysis, PD, and kidney transplantation); (2) Comprehensive care plan of a Tenchkhof catheter by a nephrologist, a convenient follow-up schedule, and medication delivery services (at the Department of Nephrology and Dialysis or at home); (3) Phone calls for periodic clinical and paraclinical monitoring of some particular patients who required remote care at home. Results: Mean age of patients was 62.4±13.0 years, 83% were elderly, and 37.7% were male. Most patients lived in rural areas (56.3%). Diabetes and hypertension were the two leading causes of ESRD (46.2% and 30.9%, respectively). The percentage of newly diagnosed ESRD patients chosing PD therapy was 12.54%. The main barriers to choosing PD therapy were small-size houses, poor hygienic conditions, and lack of family support for dialysate exchanges. Conclusion: After 2 years, the promoting PD utilization program substantially increased the rate of PD among ESRD patients. The maintaining of this program may further improve the utilization of PD in the future.
