EVALUATION OF SURGICAL TREATMENT OUTCOMES FOR UPPER TRACT UROTHELIAL CARCINOMA AT HA GIANG PROVINCIAL GENERAL HOSPITAL FROM 2015 TO 2020

Abstract
Upper tract urothelial carcinoma is a rare and malignant disease, accounting for only 5-10% of all urothelial tumors in the entire urinary tract (renal pelvis, ureter, bladder, and urethra). Although upper tract urothelial carcinoma is relatively rare at Ha Giang Provincial General Hospital, there is still a lack of consensus in diagnosis and treatment. Therefore, we conducted a study with the objective of evaluating the outcomes of surgical treatment for Upper tract urothelial carcinoma at Ha Giang Provincial General Hospital from 2015 to 2020. Subjects and methods: This was a cross-sectional descriptive study of all patients diagnosed with Upper tract urothelial carcinoma who underwent radical nephroureterectomy by open surgery at the Department of General Surgery and Oncology, Ha Giang Provincial General Hospital, from January 1, 2015 to December 31, 2020. Results: upper tract urothelial carcinoma mainly occurred in patients aged 40-60 years, with a mean age of 50.47 ± 15.29 years. The main clinical symptoms that led patients to seek medical attention were back pain (94.4%) and hematuria (80.6%). Most of the tumors were either papillary or mixed papillary and flat, accounting for 55.6% and 41.7%, respectively. The mean longitudinal size of the tumor on ultrasound was 34.7 ± 13.4 mm. 94.4% of patients underwent RNU, while 2 patients underwent simple nephrectomy and 1 patient underwent simple ureterectomy. The mean surgical time was 104.1 ± 27.6 minutes, with the shortest being 60 minutes and the longest being 180 minutes. Only one patient developed a surgical site infection, accounting for 2.8%. Of the 36 patients who underwent surgery, 34 were followed up, with 3 deaths (8.3%) and 33 surviving (91.7%). Two of 26 patients experienced recurrence in the bladder, and were subsequently treated with radical cystectomy and ureteral reimplantation. No patients showed signs of metastasis. Conclusion: Surgical treatment was effective for upper tract urothelial carcinoma at Ha Giang Provincial General Hospital.
