TREATMENT OF FRACTURE – DISLOCATION OF THE FEMORAL HEAD (PIPKIN IV) WITH INTERNAL FIXATION TECHNIQUE

Abstract

Introduction: Fractures of the femoral head combined with hip dislocation (Pipkin IV) are relatively rare and often associated with acetabular wall fractures (usually posterior wall), are indications for surgical intervention. Material and Methods: A prospective study in 25 patients with fractures of the femoral head combined with hip posterior dislocation from 6/2019. They are under surgery via Kocher – Langenbeck approach. Functional outcome was evaluated with the Merle d’Aubigne score. Results: 2 of 25 cases had small fragment, was removed fragment; 22 of them combined with acetabular posterior wall fractures. Several patients have had good outcome after 6 months, Merle d’Aubigne score got over 12 point. There were 4 patients had avascular necrosis (AVN) after 8 months, were treated with THR. Conclusion: Type IV Pipkin fractures should be treated surgically by ORIF of the acetabular fractures and fixation or excision of the femoral head fragments, reduce the anatomy and stability of the hip.
