CLINICAL, LABORATORY, AND TREATMENT OUTCOMES OF 08 TUBERCULOSIS (TB) PATIENTS WHO UNDERWENT IN VITRO FERTILIZATION (IVF) AT THE DEPARTMENT OF INTERNAL MEDICINE, NATIONAL LUNG HOSPITAL

Abstract

Objective: To assess the clinical and laboratory characteristics, diagnosis, and treatment outcomes of tuberculosis in IVF pregnant patients. Subjects and Methods: This retrospective study was conducted in the General Internal Medicine Department of the National Lung Hospital from January 2021 to March 2023. A total of 8 patients were included in the study. Results: The average age of the patients was approximately 31.6 ± 7.0 years old. All patients had fallopian tube obstruction or partial obstruction. The time from IVF embryo transfer to the onset of symptoms was 90.6 ± 21.5 days. The average time between symptom onset and X-ray examination was 23.4 ± 5.1 days. The mean white blood cell count in cerebrospinal fluid was 448.1 ± 716.1, with a higher percentage of neutrophils (50.8 ± 15.5%) compared to lymphocytes (45.5 ± 15.0%). Genexpert and Bactec culture of sputum showed a high positive rate in cerebrospinal fluid, with 75% (6) and 87.5% (7) respectively. Seven patients (87.5%) were diagnosed with mycobacterium tuberculosis. However, only 3 infants were born (37.5%), all of whom were premature and underweight. Conclusion: Genital tuberculosis is a risk factor when undergoing IVF, leading to more severe disease outcomes. Pregnant women with tuberculosis tend to experience miscarriages, premature births, and low birth weight infants.
