CLINICAL CHARACTERISTICS OF DRY EYE IN PATIENT WITH RHEUMATOID ARTHRITIS

Abstract

Purpose: To describe the clinical characteristics of dry eye disease (DED) in patient with rheumatoid arthritis (RA) and the correlation between the severity of DED with RA activity and the duration of RA. Materials and methods: a cross-sectional descriptive study was conducted in 25 RA patients with DED who were examined at the Cornea Department of Vietnam National Eye Hospital from Octobre 2022 to August 2023. Comprehensive ocular clinical examinations were performed, including an assessment of RA history, the OSDI questionnaire, ocular examination with ocular fluorescein staining, TBUT, Schirmer test. Results: A total of 25 patients were enrolled in the study with an average age of 55.32 years. All patients were female, including 22 patients with severe dry eye (88%) and 3 patients with moderate dry eye. All patients presented aqueous-deficiency, with an average Schirmer I test result of 3.86 mm, 17 patients had filamentous keratitis (68%). The average duration of RA was 12.12 years and strickly correlate with DE severity. Regarding disease activity assessed by the DAS28 score: 2 patients had high disease activity, 3 patients had moderate disease activity, 3 patients had low disease activity and 17 patients were in remission, but no correlate with DE severity. Conclusion: The DED in RA patients is aqueous-deficient as evidenced by the decrease of Schirmer I test and filamentous keratitis. DED in RA patients is more prevalent in middle age females. The severity of DED correlates with duration of RA, but does not correlate with the activity of RA.
