THE ROLE OF COMPUTED TOMOGRAPHY IN THE THE PREOPERATIVE ASSESSMENT OF ORAL CANCER STAGING

Abstract

Background: Squamous cell carcinoma (SCC) of the oral cavity is one of the most common malignancies of head and neck cancer. Study aimed to identify imaging features of SCC of the oral cavity and evaluate the value of CT in preoperative staging according to AJCC 8th. Methods: 172 cases of OSCC operated at Ho Chi Minh Oncology Hospital from March 2023 to November 2023 were included. DOIs on CECT and pathological DOI were measured. Diagnostic accuracy of the scans was calculated using final histopathology as the gold standard. Results: The study included 132 men and 40 women. The mean age in our study was 57. The tongue was the most common site with 66.3%. The mean DOI on CT was 14.03 ± 6.8mm, on pathology was 12.70 ± 5.64mm. There were 31 cases of ENE, accounting for 18.02%. DOI on CT has a strong correlation with DOI on pathology, with a correlation coefficient of 0.975 (p < 0.001). The overall diagnostic accuracy of CT in T staging and N staging is 83.72% and 76.16% respectively. CT staging has strong agreement with TNM staging results based on pathology with kappa = 0.83 (p < 0.0001). Conclusions: We suggest that with a standardized imaging protocol patients could be better classified according to CT-derived DOI.
