THE RESULT OF URETEROSCOPIC LITHOTRIPSY WITH LASER HOMIUM FOR UPPER URETERAL STONES IN BAC NINH HOSPITAL

Abstract
Objectives: To evaluate the result of ureteroscopic lithotripsy with laser Hol: YAG in Bac Ninh Hospital in managing upper ureteral stone. Subject and methods: A cross-sectional study was conducted in 128 patients with upper-ureteral stones treated with URSL by using laser Hol: YAG from June 2021 to June 2023 at Bac Ninh hospital. Results: The procedure was proceeded in 128 patients (82 males, 46 females), mean age 51,6 ± 10,9. (27,3%) 35 patients had the right ureter stones, (63,3%) 81 patients had the left ureter stones and (9,4%) 12 patients had ureter stones at both sides. The average size of ureteral stones was 10,8 ± 2,5mm. 71,8% of stones were located at the tranverse process of the L2 and L3 lumbar vertebrae. 87,5% of stones caused grade 1 to 2 hydronephrosis. 78.1% were radiopaque stones. The percentage of stone access was 93,7% and success rate was 92,2%. The average surgery time was 31,23 ± 7,9 mintues (varying from 15-62 minutes). The stone-free rate was 92,2%. The average length of stay in the hospital was 3,5 ± 1,1 days. A month after surgery, 74,2% of patients had JJ syndrome. The average time of JJ stent removal was 31,4 days. Treatment after lithotripsy: internal treatment for 7 cases of urinary tract infections and 2 cases of kidney failure; 2 patients got 2nd ureteroscopic lithotripsy; 2 patients got open surgery to remove ureteral stones, 1 case was treated with ureteroplasty and 1 case got JJ stent for urethral stricture. Normal ureteral morphology accounted for 85.2%, non-opaque stones accounted for 21.9%, with the highest rate of successful lithotripsy. Conclusion: Ureteroscopic lithotripsy for upper ureteral stones with laser Hol: YAG had high success rate (92,2%). Stone-free rate was 92,2%. Ureteral morphology and radiopaque of stone were associate factors to lithoitriosy results
