Abstract

The customized implant approach was developed by G. Dahl in 1940 [4] and is placed under the periosteum, directly on the maxillary or mandibular bone. Due to the development of digital technology, customized implants have been reintroduced with many improvements and advantages solving serious bone loss problems, making one-step surgery, shortening the number of surgeries and treatment time, and providing immediate provisional. In addition, the customized design meets the particular complex bone anatomy of each patient. In this clinical case, a 20-year-old female patient with congenital missing teeth syndrome was diagnosed with severe upper jaw bone loss, including type VI according to the Cawood and Howel 1998 classification [3]. There was not enough bone volume to place a zygomatic implant, so a customized implant was recommended. In the lower jaw, serious bone loss was observed, and the inferior nerve was exposed. Therefore, the nerve transposition was indicated with a nerve template, and All-on-4 approach was performed. Provisional was delivered immediately and the final restoration was completed after 6 months. After 6 months of follow-up, the implant was successful with no complications, significant improvement in chewing function, aesthetics, pronunciation, and improved psychological status, helping the patient to confidently integrate into the community.
