COMPARISON OF CYTOLOGICAL DIAGNOSIS WITH THYROID IMAGING REPORTING AND DATA SYSTEM (TIRADS) IN THE DIAGNOSIS OF THYROID CANCER AT THE CENTRAL ENDOCRINE HOSPITAL IN 2023

Abstract

Introductions: Thyroid carcinoma is the most common malignancy in cancer of the endocrine glands. Ultrasound-guided fine needle aspiration is a simple, low-cost, and highly effective technique in diagnosing thyroid nodules. Objectives to determine the stratification and risk of malignancy of Bethesda system’s subgroups for reporting thyroid cytopathology, in addition, compare with Thyroid Imaging Reporting and Data System [TI-RADS] in diagnosing thyroid cancer. Patients and Methods: Collect data of 1832 thyroid nodules from 1761 patients admitted at the Hospital of in National Hospital of Endocrinology from February 28, 2023 to August 16, 2023 and 424 thyroid nodules were operated. The research method is cross-sectional.. Results: The average age is 44.9 ± 12.3 years, women accounted for the majority (87.9%), thyroid nodules are mainly 10 millimeters or less, accounting for 61.6%. The percentage of ACR_TIRADS 2017 subgroups were: 0.2% (TR1), 3.6% (TR2), 11.7% (TR3), 21.1% (TR4), 63.4% (TR5) with corresponding malignant risks were: 60%(TR3), 86.9% (TR4), 96.8% (TR5). The proportion of Bethesda 2017 subgroups were: 6.4% (I), 32.4% (II), 9% (III), 1.1% ( IV), 13.6% (V), 37.5% (VI) with a malignant risk of 57.1% (I), 69.2% (II), 87.5% (III), 16.7% (IV), 98.8% (V), 100% (VI). Conclusions: The combination of US-FNAC (Bethesda 2017 classification) and ultrasonography (ACR_TIRADS 2017classification) brings high efficiency in diagnosing malignant thyroid nodule.
