EPIDEMIOLOGICAL, CLINICAL AND LABORATORY CHARACTERISTICS OF SEVERE PNEUMONIA WITH ADENOVIRUS POSITIVE SPUTUM - PCR RESULTS IN CHILDREN AGED FROM 2 MONTH TO 5 YEARS IN CHILDREN'S HOSPITAL 1

Abstract
Objectives: Describe the epidemiological, clinical and laboratory characteristics of severe pneumonia cases with adenovirus positive sputum PCR results. Subjects and methods: A retrospective cases series study was conducted concerning 55 children from 2 months to 5 years old at Children's Hospital 1 from April 2018 to March 2020. Results: The median age is 13 months, 50% of cases are less than 12 months. Males predominate with a male/female ratio of 3.2/1. Most of the cases are admitted to hospital in the winter-spring season (from October to March). The clinical presentation is quite similar to that of other viral pneumonia, with fever (94.5%) and upper respiratory tract infection (100%), but the course of diseases lasts longer with the prominent symptom of prolonged fever (9 ±5.1days). The laboratory features are nonspecific and can not be differentiated from those of bacterial pneumonia. Most of the lesions on chest X-rays are interstitial (87.3%) and bilateral (78.2%). The rate of co-infection on sputum PCR results is relatively high (78.2%), the number and the types of co-infection agents are diverse. The median number of copies of adenovirus is 40850 x103 copies. Conclusion: Severe adenoviral pneumonia in children under 5 years of age has diverse clinical manifestations, relatively similar to other viral agents but with more severe and prolonged course.
