RESULTS OF CLINICAL PHARMACY SERVICE IN VENOUS THROMBOEMBOLISM PROPHYLAXIS IN CRITICAL ILLNESS PATIENTS AT EMERGENCY DEPARTMENT, BACH MAI HOSPITAL

Abstract

Objective: This study is performed to investigate the effects of clinical pharmacist intervention in venous thromboembolism prophylaxis in critical patients at the Emergency Department, Bach Mai Hospital. Study method: An interventional study about clinical pharmacy activities on hospitalized patients at the Emergency Department. Results: 132 patients before intervention and 178 patients after intervention were included in the study. The rate of patients receiving pharmacological prophylaxis when they met the appropriate criteria increased from 48.7% before intervention to 93% after intervention (p<0.01). The proportion of patients receiving prophylaxis who had dosing adjustments based on renal function and weight increased from 76% to 90.8% (p<0.01). No significant changes in the initiation time of thrombolytic prophylaxis or the rate at which patients got full lengths of prevention according to guidelines were found. Conclusions: Clinical pharmacy activities had positive impacts on promoting rational venous thrombosis prophylaxis in critical patients at the Emergency Department. This model should be considered to apply to more subjects and to other clinical units in the hospital.
