VALUE OF XPERT MTB/RIF ULTRA TEST IN DIAGNOSING PULMONARY TUBERCULOSIS AT THAI NGUYEN LUNG HOSPITAL

Abstract

Background: Xpert MTB/RIF Ultra, also known as the Xpert super test, was deployed in Thai Nguyen to improve the efficiency of detecting and diagnosing tuberculosis. Objectives:  To determine the values of Ziehl Neelsen and Xpert Ultra in diagnosing pulmonary tuberculosis. Subjects: Patients coming for examination and treatment suspected of having tuberculosis at Thai Nguyen Lung Hospital from January 2023 - December 2023. Methods: Descriptive analysis using the entire sample. Ziehl Neelsen and Xpert Ultra test results of 422 patient were obtained with diagnosis of pulmonary tuberculosis according to standards of the Ministry of Health. Results and discussions: Of all patients with suspected tuberculosis, 87.90% had symptoms of coughing for more than 2 weeks, while 50.90% showed symptoms of weight loss. Abnormal images on chest X-ray were seen in 90.52% of the sample. The percentage of patients having fever was 73.91% and having a history of exposure tuberculosis was 15.64%. Patients with pulmonary tuberculosis accounted for 22.23%. The rate of Rifampicin-resistant tuberculosis among pulmonary tuberculosis was 6.38%. Ziehl Neelsen test had a sensitivity of 77.78%, a specificity of 97.56%, a positive predictive value of 89.74%, a negative predictive value of 97.56%. Meanwhile, those of Xpert Ultra test were 88.30%, 99.39%, 97.65%, and 99.39% respectively. Xpert Ultra has higher sensitivity than the Ziehl Neelsen test. Conclusions: Xpert Ultra was the test with the highest value in diagnosing pulmonary tuberculosis at the primary medical level.
