LONG - TERM RESULTS OF OPEN SURGICAL REPAIR FOR UNRUPTED SUBRENAL AORTIC ANEURYSM AT CHO RAY HOSPITAL

Abstract

Background: Abdominal aortic aneurysms have been described and treated for a long time. In 1951, author Dubost and his colleagues successfully operated on the first abdominal aortic aneurysm [1]. Many studies have shown that late complications during follow-up of cases after endovascular intervention to treat abdominal aortic aneurysms are high and worthy of concern [2], [3]. Author Robert Blair and colleagues (2023) studied (N=1017) the surgery results to treat infrarenal abdominal aortic aneurysms. They recorded a 30-day mortality rate of 2.5% of the study sample, with the group in the aneurysm not ruptured [5]. Author Gruber M and colleagues (2023), a comparative study between endovascular intervention and surgery for abdominal aortic aneurysm (N= 1509), recorded that the mortality rate within 30 days after surgery accounted for 2.4 % to 3% of surgical cases. Surgical treatment of abdominal aortic aneurysms is also indicated in patients who are young or have aneurysm necks that are not favorable for endovascular intervention. Methods: Retrospective description of case series. Results: The study had an average age of 64 ± 20.1 years, with the majority being male. Lipid metabolism disorders and smoking account for 90% and 85.8%, respectively. Abdominal aortic aorta fusiform lesions are the leading cause, accounting for 90% of the study sample. Abdominal aortic aneurysm with iliac artery aneurysm accounted for 85.8% of the study sample. Most use a mid-abdominal approach, accounting for 97.3% of the study sample. The study noted that Y-shaped and straight tube types accounted for 88.1% and 11.9%, respectively. Most of us included restoration of internal iliac artery revascularization, accounting for 89.6%. The aortic iliac clamping time is about 44.5 minutes, and the average surgery time is about 195 minutes. The technical success rate reached 97.2%, the 30-day mortality rate was 4.1%. The survival rate accounted for 64.8% of the study sample during the follow-up period. Conclusion: Surgical treatment of infrarenal abdominal aortic aneurysm has a high technical success rate, few complications, and a high survival rate in the follow-up period. Therefore, this method is effective, safe, and has few complications.
