THE RESULTS OF THE HYSTEROSCOPIC POLYPECTOMY IN INFERTILE PATIENTS AT NGHE AN OBSTETRICS AND PEDIATRICS HOSPITAL

Abstract
Objective: This study aimed to describe the clinical and paraclinical characteristics and assess the results of the hysteroscopic Polypectomy in infertile patients at Nghe An Obstetrics and Pediatrics Hospital. Subjects and methods: Retrospective descriptive study was conducted among infertile patients diagnosed with endometrial Polyps undergone at Nghe An Obstetrics and Pediatrics Hospital from June 2020 to June 2023. Results: A total of 79 female patients with infertility in the study included: The average age was 34.3±4.9; The percentage of primary infertility was 43.0%; Mean infertility duration was 2.8±1.5 years; 15.2% of these patients had abnormal uterine bleeding; 97.5% of endometrial Polyps were diagnosed by saline infusion sonogram; The average size of endometrial Polyps was 13.8±4.3 mm; 81.0% of cases had 1 Polyp and the majority of Polyps (34,5%) were located on the back of the uterus. Method of treatment mainly was hysteroscopy Polypectomy with endoscopic instruments (69,6%). Uterine perforation-related complications happened in 1,3% of patients. Percentages of endometrial Polyps and fibroid Polyps were respectively 86,0% and 14,0%. There were no malignant cases. The rate of patients with cessation of bleeding in the group of patients with abnormal uterine bleeding was 83,3%. The postoperative pregnancy rate was 68,0% (51/75). In the group of pregnant patients, the percentages of patients undergoing IVF, IUI and getting pregnant naturally respectively were 49.0%, 9.8% and 41.2%. The full term birth rate was 78.4%. The prevalence of preterm birth was 7.8%. The first-trimester miscarriage rate was 7.8% and the ongoing clinical pregnancy rate was 5.9%. There was no correlation observed between pregnancy rate and number, size, location, and treatment method of endometrial Polyps with p>0.05. Conclusions: Hysteroscopic Polypectomy is a safe and effective method. The uterine perforation-related complication rate was 1,3%. The postoperative pregnancy rate, the full term birth rate, the preterm birth rate, the first-trimester miscarriage rate and the ongoing clinical pregnancy rate were 68,0% (51/75), 78.4%, 7.8%, 7.8% and 5.9%, respectively. There was no correlation observed between pregnancy rate and number, size, location, and treatment method of endometrial Polyps with p>0.05.
