EVALUATE POSTOPERATIVE SYMPTOM SEVERITY AND RELATIONSHIP WITH SWELLING, TRISMUS IN PATIENTS UNDERGOING SURGICAL EXTRACTION OF IMPACTED MANDIBULAR THIRD MOLARS

Abstract
Objective: The impact of surgical extraction of impacted mandibular third molars on the quality of life (QoL) of patients has not been widely addressed in oral and maxillofacial research in Vietnam. This study aimed to assess the QoL of patients following surgery using the Postoperative Symptom Severity Scale (PoSSe) and to evaluate the relationship between scale outcomes and symptoms such as swelling and trismus. Method: This study included 64 patients who underwent impacted mandibular third molar extraction and met the inclusion and exclusion criteria. The participants’ demographic information, including age, sex, jaw section, and surgical duration, was recorded. Before surgery, a researcher measured the facial dimensions (A, B, and C) and mouth opening. On postoperative day 2, the participants returned for follow-up and the same parameters were measured. On day 7, patients completed the PoSSe scale. Data were recorded and analysed using JASP software. Results: On postoperative day 2, facial measurements A (107.88 ± 7.66 vs. 110.34 ± 7.49), B (103.16 ± 7.05 vs. 105.94 ± 6.96), and C (149.42 ± 9.26 vs. 153.06 ± 8.81) significantly increased compared to preoperative values, with p < 0.001. Similarly, mouth opening significantly decreased (451.39 ± 67.48 vs. 367.34 ± 120.23), with p < 0.001. The PoSSe scale results indicated that surgery had a notable impact on the patients' QoL, particularly affecting eating, pain, and interference with daily activities. A statistically significant association was observed between changes in dimension C and appearance as well as between reduced mouth opening and sickness. Conclusion: Surgical extraction of impacted mandibular third molars significantly affects patients' QoL, particularly in terms of eating, pain, and interference with daily activities. However, the relationship between swelling, trismus, and PoSSe scale scores requires further investigation
