TREATMENT OUTCOMES OF OSTEONECROSIS OF THE FEMORAL HEAD USING UNCEMENTED TOTAL HIP ARTHROPLASTY AT THAI NGUYEN NATIONAL HOSPITAL

Abstract

Objective: To evaluate the treatment outcomes of uncemented total hip arthroplasty (THA) in patients with osteonecrosis of the femoral head (ONFH) and analyze factors associated with treatment results. Subjects and Methods: A descriptive, cross-sectional study with both prospective and retrospective components was conducted on 47 patients (56 hips) who underwent uncemented THA from May 2023 to May 2025. Hip function was assessed using the Harris Hip Score. Risk factors, comorbidities, and postoperative complications were recorded. Results: The mean age was 53.57 ± 11.23 years. The male-to-female ratio was 3.27/1. The mean preoperative Harris Hip Score was 54.71 ± 7.21, which significantly improved to 93.68 ± 2.7 postoperatively. A history of alcohol use was found in 53.2% of patients, smoking in 23.4%, and steroid use in 8.5%. The average postoperative leg length discrepancy was 0.98 ± 0.48 cm. Postoperative thigh pain decreased over time, from 79% experiencing pain at 1 month to only 1 patient with mild pain at 12 months. No cases of infection, dislocation, stem or cup loosening were observed. One case of periprosthetic cyst was identified at 6 months postoperatively. Conclusion: Uncemented total hip arthroplasty is an effective treatment for ONFH, providing good functional recovery and a low complication rate. Alcohol use was linked to poorer treatment outcomes. Leg length discrepancy after surgery and the level of thigh pain were also identified as factors associated with treatment outcomes
