Abstract

Background: Hypospadias is a common congenital anomaly of the male urinary tract. Single-stage urethroplasty using a transverse preputial onlay island flap is widely applied in clinical practice. Objective: To describe the clinical and surgical characteristics and evaluate outcomes of urethroplasty using the transverse preputial onlay island flap technique at Children's Hospital 1, Ho Chi Minh City. Materials and Methods: A descriptive case-series study was conducted on 31 pediatric patients who underwent the procedure between April 2024 and April 2025. Results: Midshaft hypospadias accounted for 90.3% of cases; preputial hood configuration was present in 93.5%; mean urethral plate width was 5.6 ± 0.9 mm. All patients had penile coverage using local skin. The 8F catheter was the most commonly used (80.6%). The overall surgical success rate (HOSE ≥14) was 83.9% at 6 months. Conclusion: Urethroplasty using a transverse preputial onlay island flap is a safe and effective technique, providing high success rates and good postoperative functional and cosmetic outcomes with low complication rates.
