LOCAL FLAP TRANSFER IN THE TREATMENT OF LARGE CHEEK DEFECTS: A REPORT OF FOUR CASES

Abstract
Background: Large soft tissue defects of the cheek can severely impair both function and facial aesthetics. Proper selection of local skin flaps plays a crucial role in restoring contour and maintaining facial harmony. Methods: We report a descriptive case series of four patients with medium- to large-sized cheek defects treated at the Department of Burns and Plastic Surgery, Cho Ray Hospital. The defects resulted from trauma, post-oncologic resection, or infection. Local flap transfer techniques using chin–neck, lateral cervical, and preauricular skin flaps were employed to cover the defects. Clinical characteristics, surgical technique, flap survival, complications, and functional–aesthetic outcomes were evaluated. Results: All four flaps survived completely with good wound healing. No flap necrosis, salivary fistula, or major complications were observed. Oral competence, mastication, and facial expression were preserved in all cases, without significant distortion of the oral commissure or lower eyelid. Donor-site scars were well concealed along natural skin creases in the chin, neck, or preauricular area. Overall aesthetic outcomes were satisfactory according to both surgeons and patients. Conclusion: Local chin–neck, lateral cervical, and preauricular flaps are safe, feasible, and highly effective options for covering medium- to large-sized cheek defects. They provide reliable tissue coverage with good color and thickness match, while simultaneously achieving favorable functional and cosmetic results.
