EVALUATION OF THE RESULTS OF COMBINED SUPERFICIAL AND DEEP TEMPORAL FACELIFT

Abstract

Background: Temporal facelift is an effective method for rejuvenating the upper face, improving the sagging of the lateral canthal region and the tail of the eyebrow. The superficial dissection technique is simple and removes excess skin but may leave scars and have short-term results. In contrast, deep dissection beneath the periosteum lifts deeper tissue more effectively and offers longer-lasting results, though with limited mobility. Combining both superficial and deep dissection techniques aims to harness the benefits of both methods for optimal and longer-lasting outcomes. Objectives: To evaluate the effectiveness and safety of the combined superficial and deep temporal facelift technique. Materials and methods: This retrospective descriptive study included 30 patients who underwent temporal facelift surgery at the Department of Plastic and Aesthetic Surgery, Cho Ray Hospital, from March 2023 to March 2025. The procedure was performed through an incision hidden within the temporal hairline, combining both superficial and deep dissection, followed by suturing into the deep temporal fascia. Results were assessed at three time points: pre-operation, immediately post-operation, and at 6 months, using the following measures: eyebrow tail position (mm), crow's feet wrinkles (WSRS), and patient satisfaction (FACE-Q scale, 0-100). Results: Post-operatively, the eyebrow position increased by an average of +3.1 ± 0.8 mm, reaching 12.9 ± 1.0 mm, which aligns with the ideal aesthetic range according to Glass (2014). After 6 months, the position was maintained at 12.1 ± 1.2 mm (p < 0.001). WSRS decreased from 3.1 ± 0.6 to 1.2 ± 0.5 immediately post-surgery and 1.4 ± 0.6 after 6 months (p < 0.001). FACE-Q scores increased from 47.5 ± 9.2 to 79.8 ± 8.5 and remained at 72.4 ± 9.7 after 6 months (p < 0.001). The primary complications were transient swelling and bruising (16.67%) and mild numbness in the infraorbital region (26.7%), which resolved fully within 2–3 months. No permanent nerve damage was reported. Conclusion: The combined superficial and deep temporal facelift technique is a safe, effective, and durable method that lifts the eyebrow, reduces outer eye wrinkles, with well-hidden scars and low complication rates.
