SURGICAL OUTCOMES OF TUBAL ECTOPIC PREGNANCY AT BACH MAI HOSPITAL

Abstract
Objective: To evaluate the surgical outcomes of patients with tubal ectopic pregnancy at Bach Mai Hospital. Methods: A retrospective descriptive study was conducted on 60 medical records of patients diagnosed with tubal ectopic pregnancy who underwent surgical intervention at the Department of Obstetrics and Gynecology, Bach Mai Hospital, from January 1, 2024 to December 31, 2024. Results: Laparoscopic surgery accounted for 76.7% of cases, while open surgery accounted for 23.3%. The highest proportion of patients (51.7%) underwent surgical intervention within 24 hours of hospital admission. Ectopic pregnancies located in the ampullary segment of the fallopian tube were the most common, comprising 73.3% of cases. In contrast, the rate of open surgery was notably high in interstitial ectopic pregnancies, reaching 75%. Unruptured ectopic pregnancies with hemoperitoneum were the most prevalent presentation (73.3%). Ruptured ectopic pregnancies were observed in 33.3% of cases, and among these, open surgery was required in 60%. The most frequently encountered volume of intraperitoneal blood at surgery ranged from 100 mL to <500 mL, accounting for 60% of cases. The likelihood of open surgery increased significantly when blood loss exceeded 500 mL. Conclusion: urgery remains the primary and effective treatment for ectopic pregnancy. Laparoscopic surgery is the preferred approach; however, open surgery is still necessary in cases of ruptured ectopic pregnancy and hemodynamic instability.
