EPIDEMIOLOGICAL, CLINICAL, AND PARACLINICAL CHARACTERISTICS OF SEVERE AND CRITICAL ADENOVIRUS PNEUMONIA IN CHILDREN AT THE NATIONAL PEDIATRIC HOSPITAL IN 2022

Abstract
Objectives: Description of epidemiological, clinical, and paraclinical characteristics in children with severe and critical pneumonia due to Adenovirus. Methods: A retrospective cohort study including 178 PCR-confirmed adenovirus pneumonia cases treated in 2022. Severe/critical classifications followed the Vietnamese Ministry of Health guideline.1 Results: Admissions peaked in October (43.8%), with male predominance (64.6%). The 12–<60 months group accounted for 60.7%. Common symptoms were fever (98.9%), cough (98.9%), and dyspnea (90.4%). Oxygen therapy was required in 69.1% and invasive ventilation in 26.4%. Complications occurred in 27.5%. Compared with severe cases, critical cases showed lower blood cell indices and higher CRP/LDH/AST, lower protein/albumin, prolonged APTT and lower fibrinogen (p<0.05). Conclusions: Pediatric adenovirus pneumonia demonstrated high respiratory support needs and marked seasonality. Cytopenias, elevated inflammatory/organ injury markers, and coagulopathy were associated with critical illness and may assist early risk stratification
