MICROBIOLOGICAL CHARACTERISTICS AND ANTIMICROBIAL RESISTANCE OF BILE AND BLOOD ISOLATES IN ACUTE CHOLANGITIS PATIENTS

Abstract

Background: Acute cholangitis is a medical emergency with high mortality rates due to sepsis and septic shock. Furthermore, the evolving microbial profile and rising antibiotic resistance pose significant therapeutic challenges.


Objective: To analyze the microbiology and antimicrobial resistance of bile and blood bacterial isolates in patients with acute cholangitis.


Methods: A retrospective analysis was conducted on 270 acute cholangitis patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) at 108 Military Central Hospital from February 2021 to December 2025.


Results: Bile culture positivity significantly exceeded blood culture (83.6% vs. 62.8%, p < 0.001), with 37.2% showing isolated bile positivity.”E. coli, K.pneumoniae, and P.aeruginosa were identified as the predominant pathogens. Susceptibility rates to Amikacin and Ertapenem exceeded >90% for isolates from both bile and blood sources, showing no statistically significant difference (p > 0.05) whereas high resistance was observed for Ciprofloxacin and third-generation cephalosporins.


Conclusion: The combination of bile and blood cultures optimize pathogen indentification. E.coli, K.pneumoniae and P.aeruginosa remain the primary etiologic agents, exhibiting high sensitivity to Amikacin and Ertapenem.
