EVALUATION OF THE EFFICACY AND SAFETY OF NEUROMUSCULAR BLOCKADE REVERSAL WITH SUGAMMADEX IN RENAL TRANSPLANT RECIPIENTS

Abstract
Objectives: To evaluate the efficacy and safety of rocuronium-induced neuromuscular blockade reversal using sugammadex in recipients underwent renal transplantation.
Methods: A prospective study was conducted in 37 recipients undergoing renal transplantation under general anesthesia, in whom rocuronium was used for neuromuscular blockade and reversed with sugammadex at a dose of 2 mg/kg. Tracheal extubation was performed in the operating room.
Results: The mean duration of anesthesia and surgery was 182.57 ± 31.99 minutes and 143.41 ± 29.7 minutes, respectively. The total doses of rocuronium and sugammadex administered were 73.92 ± 8.9 mg and 114.32 ± 15.73 mg, respectively. The time to recovery of neuromuscular function, defined by train-of-four (TOF) ratios greater than 0.7, 0.8, and 0.9, was 2.55 ± 0.75 minutes, 2.98 ± 0.72 minutes, and 3.4 ± 0.7 minutes, respectively. All patients were extubated in the operating room at 8.31 ± 1.92 minutes after sugammadex administration, and all maintained a TOF ratio > 0.9 at 5, 15, 30, and 60 minutes post-extubation as well as in the postoperative period. No statistically significant changes were observed in heart rate, PR interval, QRS complex, or QT/QTc interval on electrocardiography (p>0.05).
Conclusion: Sugammadex at a dose of 2 mg/kg provides effective and safe reversal of rocuronium-induced neuromuscular blockade in renal transplant recipients, with no residual blockade and minimal impact on respiratory and hemodynamic parameters.
