SOFT-TISSUE MORPHOLOGY AFTER MANDIBULAR SETBACK BSSO COMBINED WITH ADVANCEMENT GENIOPLASTY IN PATIENTS WITH SKELETAL CLASS III MALOCCLUSION

Abstract
Objective: To describe changes in lower facial soft-tissue morphology and determine soft-tissue-to-hard-tissue response ratios following mandibular setback bilateral sagittal split osteotomy (BSSO) combined with advancement genioplasty in patients with skeletal Class III malocclusion.
Methods: This longitudinal descriptive study included 36 patients. Lateral cephalometric radiographs were obtained before surgery (T0), immediately after surgery (T1), and 6 months postoperatively (T2). Changes in hard-tissue and soft-tissue landmarks and lip–chin measurements were evaluated in the anteroposterior and vertical dimensions.
Results: At 6 months, the soft-tissue landmarks Stoi, Li, B′, Pg′, and Me′ showed significant anteroposterior changes (p < 0.05). The B–B′, Pg–Pg′, and Me–Me′ distances decreased significantly, with the greatest reduction observed at Pg–Pg′. The anteroposterior soft-tissue-to-hard-tissue response ratios at B′/B, Pg′/Pg, and Me′/Me were 82.4%, 60.2%, and 88.8%, respectively. The corresponding vertical response ratios were 93.9%, 81.0%, and 83.8%.
Conclusion: Chin soft tissues changed in response to the underlying hard-tissue movements, although the magnitude of response varied among anatomical landmarks. These response ratios may support postoperative soft-tissue prediction and orthognathic surgical planning.
