ANTIBIOTIC RESISTANCE PATTERNS OF KLEBSIELLA PNEUMONIAE IN ELDERLY PATIENTS WITH PNEUMONIA AT THONG NHAT HOSPITAL

Abstract
Objective: To analyze the epidemiological characteristics and evaluate the antibiotic resistance levels of Klebsiella pneumoniae strains causing pneumonia in the elderly. Methods: A prospective, cross-sectional descriptive study was conducted on 293 inpatients aged ≥ 60 years treated for K. pneumoniae pneumonia at Thong Nhat Hospital from January 2023 to October 2024. Results: The median age was 81 years, with 67.2% being male. The prevalence of multidrug-resistant (MDR) strains was 38.6%, significantly higher in hospital-acquired pneumonia (HAP) at 58.0% compared to community-acquired pneumonia (CAP) at 26.5% (p < 0.05). Carbapenem-resistant K. pneumoniae (CRKP) accounted for 29.7%. Infections with MDR and CRKP strains elevated mortality risk by 2.74 and 2.95 times, respectively. The bacteria exhibited >50% resistance to most beta-lactams and quinolones but remained highly susceptible to Amikacin (1.5% resistance) and Colistin (10.0% resistance). Conclusion: K. pneumoniae in elderly patients exhibits alarmingly high multidrug resistance, particularly in hospital settings. Amikacin and Colistin remain among the few effective therapeutic options.
