EVALUATION OF INITIAL RESULTS OF UNILATERAL BIPORTAL ENDOSCOPIC DECOMPRESSION SURGERY IN DEGENERATIVE LUMBAR SPINE DISEASE IN PATIENTS OVER 60 YEARS OLD

Abstract
Objectives: Evaluating the effectiveness of unilateral biportal endoscopic surgery for the treatment of lumbar spondylosis in patients over 60 years of age.
Methods: This retrospective study examines 24 patients with herniated discs and lumbar spinal stenosis who underwent unilateral biportal endoscopic surgery at STO Phuong Dong Hospital from April 2025 to February 2026.
Results: The average age was 67.88 ± 5.69, with a female/male ratio of 2. The common location of the lesion was at L45, and spinal stenosis was the most common cause, accounting for 58.4%. Hypertension was the most common underlying medical comorbidities, accounting for 37.5%. The average surgical time was 81.04 ± 40.27 minutes, and the average blood loss was 30.5 ml. One patient experienced leg weakness after surgery, and one patient experienced recurrence requiring revision. The average walk-through time after surgery was 4.96 ± 1.55 hours, and the average hospital stay was 19.1 hours from the time of surgery. Bilateral decompression surgery was the most common procedure, accounting for 70.8%. There was a significant improvement in back and radicular pain (VAS) (5.62±1.23 and 7.96±0.36) compared to the time to ambulation (2.13±0.68 and 1.33±0.56), and continued improvement at 1 month (1±0.79 and 1.08±0.83) and after 3 months (0.8±0.14 and 1±0.62). Good and very good results according to Macnab were observed in 83.3% of patients.
Conclusions: Unilateral biportal endoscopic surgery in patients over 60 years old is a safe and effective procedure that may be considered a promisingsurgical option.
