OUTCOMES OF SURGICAL EXCISION COMBINED WITH SUPERFICIAL RADIOTHERAPY IN THE TREATMENT OF THORACIC KELOID ON AFTER OPEN-HEART SURGERY

Abstract

Objectives: To evaluate the outcomes of surgical excision combined with superficial radiotherapy for keloids on chest after open-heart surgery.


Methods: A prospective descriptive study was conducted on 16 patients with keloids on chest after open-heart surgery who were treated at the National Burn Hospital between 2021 and 2025. All patients underwent complete keloid excision and primary suture. Using the SRT-100 system to take radiation on the suture within 24 hours after surgery, with a total dose of 18 Gy delivered in three fractions. Treatment outcomes were assessed using the Patient and Observer Scar Assessment Scale (POSAS) and recurrence rates after a minimum follow-up period of 12 months.


Results: The mean age was 43.8 ± 11.9 years, and 62.5% of patients were female. All patients had received previous treatment and subsequently experienced recurrence. After 12 months, the total POSAS score decreased from 81.8 ± 5.0 to 39.9 ± 5.8, representing a 51.2% improvement. The observer POSAS score decreased from 40.1 ± 3.9 to 19.6 ± 4.0, while the patient POSAS score decreased from 41.7 ± 3.1 to 20.3 ± 4.2. The recurrence rate was 25.0%.


Conclusion: Surgical excision combined with SRT-100 radiotherapy significantly improved scar quality and reduced recurrence in patients with keloids after open-heart surgery.
