MAGNETIC RESONANCE IMAGING VALUE IN THE ASSESSMENT OF TRAUMATIC ANTERIOR CRUCIATE LIGAMENT TEARS AT THAI NGUYEN NATIONAL HOSPITAL

Abstract

Objective: To describe the magnetic resonance imaging (MRI) features of traumatic anterior cruciate ligament (ACL) tears and to evaluate the diagnostic value of MRI in the diagnosis of ACL tears, using knee arthroscopy (AS) as the reference standard.


Subjects and methods: A descriptive cross-sectional study combining retrospective and prospective data was conducted in 77 patients with traumatic knee injuries who underwent preoperative knee MRI and AS at Thai Nguyen National Hospital. AS findings were used as the reference standard. Direct and indirect MRI signs, tear location, and tear severity were assessed. Diagnostic performance was evaluated using sensitivity, specificity, positive predictive value, negative predictive value, accuracy, and Cohen’s kappa coefficient.


Results: The mean age was 37.82 ± 13.56 years, with the 20–40-year age group accounting for the largest proportion (49.4%). The male-to-female ratio was approximately 1.26/1. The most frequent direct MRI sign was increased ACL signal intensity on T2-weighted and PD fat-suppressed sequences (74.0%), followed by discontinuity of the ligament (70.1%). The most common indirect signs were joint effusion (97.4%), bone marrow edema involving the femoral condyle and/or tibial plateau (64.9%), and posterior cruciate ligament buckling (59.7%). Type II tears were the most common location on MRI (33.8%), followed by type III tears (27.3%). Compared with arthroscopy, MRI showed a sensitivity of 98.67%, specificity of 100%, positive predictive value of 100%, negative predictive value of 66.67%, and overall accuracy of 98.70% for detecting ACL injury. The accuracy and kappa coefficient for tear location were 78.1% and Kappa= 0.692, respectively. For tear severity, MRI achieved an accuracy of 89.6% with a kappa coefficient of 0.774. Sensitivity was lowest for low-grade partial tears (60%), whereas complete tears showed a sensitivity of 92.7%, specificity of 90.9%, and accuracy of 92.2%.


Conclusion: 1.5 Tesla MRI provides high diagnostic value for traumatic ACL injury, particularly for detecting the presence of injury and complete tears. MRI demonstrates good agreement with AS for determining tear location and severity; however, its ability to detect low-grade partial tears remains limited.
