MULTIDETECTOR CT IMAGING FEATURES OF INTESTINAL AND MESENTERIC INJURIES IN VIETDUC HOSPITAL

Abstract

Objectives: Study the features of multidetector CT scan in the diagnosis of  intestinal and mesenteric injuries from blunt abdominal trauma. Subjects and methods: A cross-sectional descriptive study of 40 patients diagnosed with intestinal and mesenteric injuries on multidetector CT scan and operated at Viet Duc hospital from 7/2019 to 6/2020. Surgical results have 35 patients with intestinal and mesenteric injuries and 5 cases with other visceral injuries. Results: The mean age of the patients was 32.2 ± 13.8 years old. On CT scan, free abdominal fluid was the most common sign with 35/35 (100%) cases, and free abdominal air was 27/35 (77.1%). Intestinal wall thickening, abnormal intestinal wall perfusion and discontinuity were 27/35 (77.1%), 17/35 (48.6%) and 7/35 (20%). Mesenteric fat infiltration were 20/35 (57.1%), mesenteric hematoma was less common with 6/35 (17.1%) and mesenteric extravasation was 4/35 (11.4%). In terms of diagnostic value, intestinal wall thickening and abnormal perfusion have high sensitivity (77.1% and 80%) in diagnosing intestinal and mesenteric injuries. The specificity (100%) and sensitivity (77.1%) of the free abdominal air are highly reliable (p<0.01). Retroperitoneal air was a significant negative sign in diagnosing intestinal and mesenteric injuries (p<0.01). Conclusion: Multidetector CT scan are reliable method in diagnosing intestinal and mesenteric injuries.
