Abstract
Research objective: Early results of distal subtotal gastrectomy, D2 lymphadenectomy, Billroth I anastomosis in the treatment of gastric adenocarcinoma, at the Department of Abdominal Surgery 1, K Hospital. Methodology: Retrospective combined with prospective description of 30 patients with gastric adenocarcinoma, underwent a distal subtotal gastrectomy with D2 lymphadenectomy, Billroth I anastomosis from January 2021 to August 2022 at the Department of Abdominal Surgery 1, K hospital. Result: Through an evaluation study in 30 patients, we found that the age group over 60 years old was the most common, accounting for 53,3%, the incidence in men is higher than in women, accounting for 60%. The average operation time was 143 minutes, the shortest was 120 minutes, the longest was 200 minutes. An average of 22,03 lymph nodes were removed, lymph node metastasis in N1 and N2 group, accounted for a high rate of 36,7% and 23,3%, respectively. The majority of the disease is stage III, accounting for 66.7%. The average time of gastric tube removal after surgery is 1-2 days, feeding initiation after surgery on day 2nd or 3nd, average hospital stay of 7-8 days. There was 1 case of early postoperative complications (accounting for 0.33%) with pneumonia, patient was treated by medicine, then stabilized and discharged after 14 days. Conclusion: Distal subtotal gastrectomy with D2 lymphadenectomy is safe surgery, feasible, can be performed at any disease stage, low complication rate, shorten surgery time and intraoperative blood loss.
