PAPILLARY MUSCLE RUPTURES DUE TO MOTOBIKE ACCIDENT: A CASE REPORT

Abstract
An 18-year-old teenager admitted to the department of emergency in a critical condition due to a motorbike accident. On admission, he was in coma, intubated, his blood pressure was at 50/30mmHg, oxygen saturation was 80%, and multiple traumas. After auscultation, we revealed clear systolic murmurs and point-of care ultrasound detected a rupture of the whole mitral valve apparatus which led to cardiogenic shock. After a quick heart team discussion, we decided to put on veno-arterial extracorporeal membrane oxygenation (VA ECMO) and followed by mitral valve replacement. Operative findings showed that the mitral valve papillary muscles are totally damaged. Post operative progress was critical due to lung hemorrhage. Eventually, the patient died at the third day of ICU stay.
