Abstract

Postoperative chylous fistula. (CF)is a rare complication of retroperitoneal surgery. CF can occur after abdominal aortic surgery or retroperitoneal lymph node dissection. This rate is higher in those who have undergone lymphadenectomy in patients with a radical nephrectomy, and more often on the left side than on the right. There is currently no standard treatment for CF. Imaging or surgical intervention is usually given if conservative treatment fails. We report a case of a patient undergoing radiographic intervention for thoracic catheterization due to narrowing of the thoracic outlet into the left subclavian vein in the treatment of chylous fistula. This is a patient after extensive left nephrectomy for renal cell carcinoma at Hanoi Medical University Hospital. Result: Narrowing of the thoracic outlet into the subclavian vein increases lymphatic pressure, is associated with abnormally enlarged abdominal lymph nodes, and is a risk factor for postoperative lymphatic fistula. Imaging or surgical intervention is necessary if conservative treatment fails. Catheterization in the treatment of thoracic duct stenosis safely and effectively reduces lymphatic.
