THE ROLE OF MAGNETIC RESONANCE IMAGING IN THE DIAGNOSIS OF ADNEXAL MASSES IN BORDERLINE OF FROM BENIGN AND MALIGNACY

Abstract

Background: Ovarian tumors in borderline of benign and malignancy are a major challenge to clinical practices. Early detection and precise diagnosis might help to provide a good management approach. This study is aimed to assess the effectiveness of using Magnetic Resonance Imaging (MRI) to identify benign and malignancy of adnexal masses. Objective: To identify the diagnostic value of Magnetic resonance imaging in diagnosis of ovarian cancer in adnexal masses unclassified by the IOTA Simple Rules. Methods:  A retrospective cohort is carried out by retrieving all MRI images database from 1116 women with undetermined ovarian tumors at the Tu Du hospital between the 1st of January 2019 and the 31st of March 2020. Histopathologic result is considered as a gold standard of accuracy by using MRI. Results: A total of 246 women who performed MRI, the overall accuracy for diagnosis of malignant ovarian tumors is 83.9%. The sensitivity, positive predictive value, and negative predictive value of MRI in predicting ovarian cancer was 84% (95% CI 75%-91%), 70% (95% CI 60%-78%), and 92%, respectively. The MRI findings are most predictive of malignancy were vegetations in cystic lesion (odds ratio, 8.6), heterogeneous enhancement patterns (odds ratio, 8.3) and Ascites(odds ratio, 15.4). Conclusions: MRI is helpful to determine benign and malignat adnexal masses in case of sonographical imagines indiscrimination.
